Presenting Clinical Problem

A 52-year-old patient from the Clayton area presented with multiple clinical concerns impacting both tooth structure and gingival balance.

• Enamel Cracks: Vertical fissures were observed on several upper anterior teeth, compromising surface integrity.
• Gingival Overgrowth: Uneven gum margins created asymmetry across the anterior maxillary arch.
• Tooth Wear: Reduced incisal height and visible flattening were consistent with long-term functional wear.
• Mild Crowding: Misalignment of upper anterior teeth limited restorative options without a cosmetic approach.

The primary clinical objective was to restore structural integrity, correct gingival asymmetry, and re-establish functional and aesthetic harmony in the upper anterior region.

Treatment Plan & Clinical Process

A multi-phase treatment plan was developed to address the patient’s requirements and ensure predictable outcomes.

1. Phase 1: Diagnostic & Planning
   Intraoral scanning and Digital Smile Design were used to evaluate the presenting issues and develop a virtual treatment plan. Photographs, radiographs, and occlusal analysis were obtained to guide decision-making and align restorative goals.
2. Phase 2: Gingival Recontouring
   Laser gum contouring was performed to reshape irregular gingival margins. This procedure established a balanced gum line, providing a stable foundation for restorative work and improving symmetry across the anterior dentition.
3. Phase 3: Definitive Restorations
   Eight porcelain veneers were designed and placed on the upper anterior teeth. These restorations were fabricated to restore lost incisal length, reinforce areas weakened by cracks, and create proportional alignment. Layered ceramics were used to replicate natural translucency and shade, ensuring the veneers blended seamlessly with adjacent teeth.

The treatment was completed over two visits, with careful attention to maintaining periodontal health throughout.
